Follow-up Care after Laser Eye Surgery
Contact Lens Policy
In order to have a successful laser refractive surgery, contact lens wearers must refrain from wearing their contact lenses for a minimum time period prior to their diagnostic evaluation. Contact lenses can change the corneal curvature and that will affect the measurement of your refractive error. To achieve the best correction with laser refractive surgery your cornea must stabilize and return to its natural shape. This will give a more accurate assessment of your refractive error.

Before Eye Surgery
At Eyetech Lasik Clinic we need to satisfy ourselves that we are assessing the health of your entire eye – not just your refractive error.
At your pre-operative assessment we will do an Orbscan™ to map the front and back surfaces of the cornea. This is important because some conditions like Keratoconus are apparent on the back surface of the cornea first. Keratoconus is a condition where refractive laser surgery is not indicated. Topography machines only map the front surface of the cornea and therefore could miss an early case of Keratoconus.
The Orbscan™ will also measure the amount of corneal tissue available for surgery. The Orbscan™ give measurements of corneal thickness in any part of the corneal as opposed to ultrasound machines that measure corneal thickness only where the probe touches. In some cases there may not be enough corneal tissue available for LASIK surgery and PRK may be an alternative procedure. In cases where the corneal tissue is borderline, an ultrasound measurement will also be taken to confirm the Orbscan™ findings.
WASKA Wavefront Analizer:
This instrument is used to analyze optical imperfections in the eye that may cause problems in dim light or at night. This test is done in the dark to imitate the condition in the dark. It analyzes not only your prescription but also detailed wavefront analysis in 12 milliseconds–the fastest in the market. It creates the basis for customizing your unique and precise correction for your eyes.
A pressure test will be done to check for glaucoma. Dr. Kadambi will do a full front to back eye examination, including a dilated pupil examination to check the health of the retina and its associated structures. After the pupils are dilated he will do another refraction. Your pupils will be measured in normal and dim lighting conditions. If, for any reason, laser refractive surgery is not in your best interest you will be advised accordingly.

During Eye Surgery
On your day of surgery, you will be asked to arrive one hour before the scheduled procedure. You must not put any eye makeup or creams, or use perfume or after-shave on the day of surgery. Please wear warm clothing as the operating room is quite cool. Make sure you bring a friend or relative to take you back to the hotel or home after the surgery. You need to be in the center for an hour or so.
The surgical counselor will review the surgical consent forms with you and prepare them for signing. Your post-operative care and your medication will be discussed with you as well.
Dr. Kadambi will see you prior to your surgery. He will check your vision one more time prior to the operation. He will go over the consent form with you and you will sign it in his presence. Any additional questions you may have will also be answered at this time. Then one of the staff will prepare you for the surgery.
The LASIK procedure itself is a brief, relatively painless procedure. No sedative is given, as your co-operation during the surgery will be needed-especially to fix your stare on a light during the short period when the laser is working. No drops will be put in your eye till you are already on the surgical bed under the microscope.
Before beginning the surgeon will call your name and check the correction he is doing on the eye with the surgical assistant. Usually surgery is performed on the right eye first. A drop of anesthetic is put in your eye and your eye lashes are covered. A "speculum" is inserted to keep your eye lids apart to expose the cornea. A dye marker is made on the cornea to help the surgeon align the flap after the surgery. Next the surgeon will make the flap.
To make the flap:
If you choose to have the flap done by IntaLase you will lie down on the bed and an anesthetic drop is instilled your eye. The eye is epopee by a delicate instrument called speculum. The other eye is covered. A suction ring is applied to hove your cornea exposed properly and the intralase equipment is adjusted to the suction ring. The laser is then delivered to create the flap at the precise depth by the surgeon. it tales less than 30 seconds for the creation of the flap. The suction is released and the flap is created in the other eye. After the flaps ar3e done in both eyes you will be transferred to the the other bed for doing the Laser vision correction. The Flap is lifted and the laser vision correction is done–First in one eye and then the other eye.
If you choose to have the XP keratome to create the flap :A drop of anesthetic is placed in your eye and speculum is applied It will take less than a minute. A suction ring is then applied and you will feel a sensation of pressure in the eye. You will lose the sight of the light in that eye for a very short period. The microkeratome makes the flap and you will hear a buzzing noise. The suction is released and the microkeratome along with the suction ring is removed. The flap is then lifted and held back to expose the corneal "bed". The laser is then "locked" and started. You will hear a tapping noise. You may smell a "burnt hair" type of smell. The laser delivery is often done in few "Zones" with only seconds between the zones.
Once the laser is done the flap is put back to its original position and aligned properly after washing under the flap. Antibiotic drop is instilled and the speculum and the tapes are removed. You will be able to blink normally once the speculum is out. The whole process usually takes 3 to 4 minutes. The surgeon then proceeds to do the other eye and the whole process repeated for the other eye.
After both eyes are treated you will be helped out of the bed and you will be taken to the postoperative room. You can rest for a while and when you feel ready to go inform the staff. Remember your vision will be blurred all that day. The discomfort you feel after the surgery will improve after time. Before you go make the appointment to see the surgeon the next day.
PRK procedure is similar to LASIK except there is no suction or using the microkeratome. After the lid speculum is applied a circular mark is done on the cornea to show the area of removal of the "epithelium" so laser can be done on the corneal tissue. The laser correction part of the procedure is done the same as in LASIK. A contact lens is inserted at the end of the procedure to minimize the pain post-operatively. The contact lens stays in the eye till the epithelium covers the bared cornea-usually 3 days. Patients having PRK experience more discomfort than ones do that had LASIK.
The next day the surgeon checks your vision and inspects the flap to make sure the flap is in position. You will be required to be followed-up in 1 week, 1 month and 3 months either at Eyetech or with your own optometrist. Your optometrist will be given all the relevant information and he/she will inform us your progress on a regular basis.
Follow-up Care after Laser Eye Surgery
As with any surgical procedure, follow-up care is an essential part of patient recovery after laser refractive surgery. Patients will be assessed at the day after the operation by the surgeon. This is a mandatory follow-up in the clinic. All follow-up care can be done at Eyetech Lasik Clinic at no additional charge to the patient.
Other follow-up assessments must be done at one week post-operatively, and one month and three months post-operatively. These post-operative visits can be done with a co-managing doctor if the patient wishes. If the patient chooses to go elsewhere for follow-up care, we will send a report to the doctor of their choice. The doctor in turn will send Eyetech Lasik Clinic all of the follow-up reports about the patient. This way we can keep informed about the patient’s progress. These progress reports from co-managing doctors assist us in monitoring outcomes for the sole purpose of ensuring your best visual result.
